COVID-19 Screening Form

If you have tested positive for COVID-19

If you test positive for COVID-19, Day 0 is your first day of symptoms or a positive test result (if asymptomatic). Day 1 for determining isolation time is the first full day after symptoms developed or the date of a positive test.

Employees: stay home from work and contact your immediate supervisor and the Office of Human Resources (hr@harding.edu).

Students: remain in your room and contact the Office of Student Life (studentlife@harding.edu) and Student Health Services (SHS) (studenthealth@harding.edu).

Isolate from others for 5 days. Students are allowed to isolate themselves in their own room.

If you had symptoms, end isolation after 5 full days from symptom onset if symptoms have improved and you have been fever free without the use of medication for at least 24 hours. You will need to wear a well-fitting mask for an additional 5 days.

If your symptoms are not improving, please contact studenthealth@harding.edu.

If you did not have symptoms, end isolation after at least 5 full days after your positive test. CDC recommends wearing a well-fitting mask for an additional 5 days.
